MAF issues

Post MAF issues 
I keep getting a MAF code, when i have it checked i dont remember the code number but it always said High output low voltage. it used to do it all the time until switched out the MAF ( it didnt help any) and re wired the the wires back to the computer. Now it only throws the code sometimes when i go past 5,000rpms when it does it it basically cuts the fuel to the truck and i cant give any gas or my afr will go way lean. Also it has trouble idling sometimes once the code is thrown.

I have also swapped computers to see if anything changed but nothing did. i have the passenger side valve cover vent plumbed into the air intake pre blower right after the MAF just like stock was that couldnt be my issue could it?

it was tunned in person by nelson performance and dont think it has anything to do with the tune.

Does it have a Blow-Off valve? If so, the expelled air needs to go back in---post MAF/Pre Boost.
There is not much to the wiring of the MAF. If it is a 3 wire, Then Pink is 12Volts in, Black/Wht is Ground, and Yellow is Signal back to the MAF. If it is a 5 wire, then the other two wires are for the IAT sensor portion of the MAF.

It is a whipple on a 1997 vortec 350 it circulates the are air when not in boost and that is post MAF/Pre boost. It is the thre wire and im sure i wired it back correctly, I put a new plug in on then wired the yellow back into the computer, then grounded the black/white, then took the pink to a 12v source. it is better than before but it still throws the code sometimes
