Do you have any fuel pressure and volume measurements? Also have you been able to access any fuel trim info? Since you have mentioned that new fuel pump and filter fixed similar symptoms in the past, that is where I would suspect the issue first too. Also the connector issue as mentioned by Speeder....

Just wanted to update. The hard start issues turned out to be coming from a intermittent fault in the ignition switch, which failed to turn on all circuits and as a result dash was not lit for a few seconds while cranking. However, replacing the fuel pump apparently fixed another long over due issue...

They always used that "disclaimer". EPA probably asked for their sales records to prove it and they either didn't provide them, couldn't provide them, or otherwise failed to prove their point of "off-road use only". EPA has busted several big tuner hardware manufacturers over the years put them out...
